Description Assessment Purpose: The student will be able to: • Write a medication profile on a specific medication. • Write a cultural assessment highlighting the client’s use of traditional or alternative medicine. • Write an education plan for a patient prescribed a specific medication from a scenario. • Deliver the education plan to the patient. Assessment Task/Activities: Medication Profile and Patient Education Plan • Each student will be allocated a scenario for a patient with a prescribed medication. SCENARIO 1: MEDICAL Mrs Taufa is 81 years old. She is admitted to an acute medical ward with chest pain, and is diagnosed with unstable angina. She and her husband live with their eldest daughter and her husband and 3 children. She has a history of hypertension, myocardial infarction and Type II diabetes. She still feels very tired and gets a tight feeling in her chest when she walks to the toilet. Her current medications are: Cilazapril 5mg PO mane Lilly • Research your allocated medication and complete a medication profile using the criteria in the marking schedule. DO NOT just copy and paste from websites (written). • Describe the cultural assessment you will do to identify the patient’s current use of traditional or alternative medicines. • Using the guidelines from your medication administration lectures, provide an education plan (using the template provided). Aim to ensure your patient is assessed and fully informed to be able to safely self-administer this medication at home. • Demonstrate the delivery of an education session, using your plan, to enable the patient to safely self-administer the prescribed medication at home. (Practical) • Administer prescribed medication safely to your client. (Practical) • You may use any appropriate resource to support your education plan and can use this resource to educate your patient.
