Must use these books: 1.Instructions of Students with Severe Disabilities by Brown, McDonnell, and Snell 2. Transition Education and Services for Students with Disabilities (5th edition) by Patricia Sitlington, Debra Neubert, & Gary Clark Develop a Post-Secondary Transition Plan designed to create an ideal setting for individuals to live or work as independently as possible in a supported, integrated setting at least 5-10 years after high school graduation. Include an examination (using citations to support your content) of current transition issues regarding vocational or independent living skills. Go for a big and bold vision of what you think should be in place for individuals with disabilities! Your plan should go beyond what is typically offered for students with disabilities as they go on to post-secondary life and creatively describe a “utopian” workplace or residential setting that clearly demonstrates your knowledge about what is really needed for these individuals to have a more inclusive, personally satisfying quality of life. Work should be original! Choose to develop an original creative plan for the workplace OR residence. Organize your plan using these components as topics bolded within your document in the following order: Introduction: Include an engaging introduction explaining your ideal post-secondary/transition setting and an inspiring reflective summary. Innovation in Action: Give specific, innovative examples and a clear description of your ideal workplace or residential setting, which can include breaks, recreation-leisure activities, access to personal needs, transportation, daily schedules and other important adult lifestyle issues. Building Friendships: Give specific examples and creative ideas about how individuals can develop an array of friendships and social skills in your ideal setting, including relationships with persons who are non-disabled, and explain how they might maintain these friendships over time, including family relationships. Technology/Assisted Technology: Give specific examples of how individuals with disabilities are supported in your ideal workplace or residence and how they can use IT/AC and other adaptations (technology, peers/co-workers, natural supports) to gain access to personal choices and success. Building New Skills: Discuss how new skills can be learned in your ideal setting (i.e. new job skills, adapting to new friends, new responsibilities) and can help them achieve self-advocacy skills and their own dreams. Realities of Work: Select what you feel are the three most important realities of work, and defend your selection of each. Discuss how you convey each of these realities to your clients. ***Document must be APA compliant. A minimum grand total of 7 pages of content. The title, abstract, and reference pages will make the entire document a grand total of 10 pages.***
