CHHS 496B: CAPSTONE REPORT – MILESTONE Please use the APA formatting guidelines for all parts of this report. Please create this assignment as a Google Doc and share with your Capstone Instructor. Capstone reports will be written in a narrative format (not in a question-answer or bulleted outline). Purpose: This report will be a year-long process that will be finalized after implementation of your capstone project. At all Milestones the following must be included: • APA Cover Page • Reference list: Please include valid research sources using APA style (~ 8). • • Appendix: Include Scope of Work, any supplemental materials, e.g., copies of questionnaires, flyers, educational materials, etc. Project results (2 – 3 pages) – Expected to be completed for Milestone 3 A. Project activities a. What program activities were actually conducted (this may differ from the Scope of Work submitted for Milestone 2) and describe the overall implementation process. B. Project outcome(s) . What are the expected project outcomes that were determined by you and your mentor to be achieved by the conclusion of the project? C. Findings/results Discuss what happened as a result of your project and whether and how it achieved the expected outcome(s), providing evidence from your assessment (use tables and charts in this section and make sure there is sufficient explanation of the data). 1. Describe the outcome measure(s) used to assess the effectiveness of your project’s progress toward achieving the outcome. 2. Describe the data collection method used to gather evidence to measure the project outcome. 3. If the project achieved the expected outcome(s), discuss how and why. 4. If the project did not achieve the expected outcome(s), discuss why not, as well as what it did achieve.
