Describe the issue and why it was a problem. What would be the end goal and how can you measure that it was met? For example, there was a number of hip dysplasias in the community and my workplace changed the method for nurses to swaddle the newborns and teach families to help reduce the incidence. Describe and use a quality improvement model such as the Model for Improvement (Plan-Do-Study-Act cycles), Six Sigma, Lean, Root Cause Analysis, Care Model, and FADE. Describe the steps you would take to tackle the issue using the model you have chosen to guide you.
