Completed research and identified three sources to use for the E/M Coding In-Service Training. Now, you will apply your instructor’s feedback to your sources and create a checklist for E/M coding tools. This will give you an opportunity to get your instructor’s feedback on what should be included in your E/M coding tool before you submit it as part of your in-service training. As a reminder, your E/M Coding Tool must help the front office staff complete the following tasks when coding: • Identify the history of the chart being coded • Identify the exam of the chart being coded • Identify the medical decision-making of the chart being coded • Identify the contributory factors, if applicable, to assign the appropriate codes For each key component and the contributory factors, the tool must clearly explain what needs to be included to select the correct level of the E/M service.
