For guidance, refer to the APA manual (reference below) and The OWL at Purdue, APA Formatting and Style Guide (http://owl.english.purdue.edu/owl/resource/560/01/) Instructions: As a human service professional, the ability to identify and respond to the presenting crisis and prevention needs for the populations we serve is vital. For this assignment, you will develop a prevention and intervention plan for ONE hypothetical client that presents with a crisis and/or prevention need. When completing this task, you should write your plan as if you are a human service professional working in your local community. Acting in this role, you are creating a prevention and intervention plan to a treatment team of helping professionals who will assist you in implementing your plan. Your agency provides service linkages to a variety of resources related to counseling, mental health and addiction, financial management, parenting, and emergency housing. Within your plan please be sure to include the following components: 1. An overview of your chosen case and presenting needs a. Provide an overview of your client’s demographics b. Be sure to discuss both the crisis and prevention needs of the client 2. Incorporation of a Crisis Intervention Model (chapter 1 of Gilliland text) a. Identify the type of crisis your client is experiencing b. Select one of the crisis intervention models presented in the text and discuss how it would be best suited to address the type of crisis your client is experiencing. c. Identify a specific intervention resource in your community that would be best suited to address the needs of your client 3. Identification of Prevention and Intervention Resources a. What referrals would need to be made for the client? b. Identify a specific prevention resource in your community that would be appropriate for your client. 4. Discussion of potential risk factors a. Identify a minimum of 2 risk factors that could manifest throughout the service delivery b. Be sure to include an explanation as to why these specific factors were identified as being a risk 5. Discussion of Ethical Issues a. Considering the risk factors previously identified for your client what are some ethical considerations for this case? b. Be sure to support your answer with applicable ethical standards from the National Organization of Human Services (2015).
