Community Assessment.
You are expected to write double-spaced paragraphs for the community description, data analysis, and summary sections of this assignment. As usual, you should use correct grammar, spelling, and syntax. In addition, you should use in-text citations appropriately and include a reference and title page as you will see in the template outline. You may use bulleted lists when appropriate; i.e., in collecting information on social support agencies, you may list these items. Keep in mind, that this list should form the basis of a referral list for you to utilize in your clinical rotation. We hope that you construct your lists with that thought in mind. It is suggested that you add a few words after the name of that social support agency so that in the future when you refer to that list, you will know the services offered by a specific agency. At a later date, you will need to add contact information and eligibility guidelines for specific agencies. It will not be helpful if you refer a victim of domestic violence to a shelter only for her to find out when she arrives there that either her children or herself are not eligible to stay. Step one: General Description of Community You should begin with a description that should include the following: General Description of [my community] Location (County where the community is located) Interesting topography or environmental conditions that could affect access to care, i.e. extreme weather conditions, low lying area, lack of OB providers etc. Local transportation systems-are they any discount programs for low-income women to access care? Major employers-professional? —blue collar, seasonal (how may seasonal or part-time employment impact health insurance coverage?) Social service resources for pregnant women–who might you want to partner with as a CNM? Don’t forget to include WIC. Home health agencies that provide maternal–child health services-eligibility requirements? Women’s and family shelters–eligibility requirements? Step two: Complete community assessment table Using the web resources provided in the External Resources folder (and any other web-based resources you need), conduct a community assessment (of the community in which you plan to work) by completing the table within the paper template found above. Please remember to use correct statistical descriptors in your work. Pay particular attention to reporting percentages (rather than rates) on your tables. You must clearly indicate the year and source for each area of your data. In-text style citations should accompany your table. A key to identify the source of each citation is strongly encouraged. All of the citations used for your table must be fully listed below the table and some within your references. Do not forget to indicate your community and state on the top of your data table.
