PHA6632 Foundations of MTM II

  • n Planning a Model (answer the following questions)
  • n ØIf there is no perceived or apparent demand or need for an MTM service, how does one go about creating or identifying a need?
  • n ØWhat online resources may help?
  • n ØWhat existing in-house pharmacy resources may help?
  • n ØWhat other techniques could be used to identify a need?
  • n Operational Procedures – briefly describe the operational procedures for one of the models listed below. (The assignment may be better performed with a table format)
  • n ØList the procedures and who should perform them
  • n ØHow will the service be marketed?
  • n ØHow will scheduling, billing and documentation be handled? Workflow?
  • n ØDescribe step-by step the work flow process from beginning to end for each model. Include billing codes if applicable, software type, application and how it will be used

 

  • n Clinical Training of Existing Staff
  • n  How does one match the training to the service? Select one type of service and describe e.g. monitoring, CMR, Pharmacotherapy, Alternative Medicine, Health and Wellness
  • n What training (if any) would each of the individuals listed below need?
